A company is looking for a Licensed Practical Nurse (LPN) with
Massachusetts Licensure required.
Key Responsibilities
Manage patient census to close gaps in clinical and non-clinical
patient care
Identify and coordinate community resources for patient care
Provide education on chronic condition management and perform
medication management
Required Qualifications and Education
Active Massachusetts License (LPN)
At least 3 years of direct patient-facing work experience
Technical aptitude with Microsoft Office Suite
Experience with medication management and chronic care
coordination
Ability to work in a remote, HIPAA-compliant workspace
